The registered nurse case manager will be responsible for managing and coordinating all care for you and your family including assignments and schedule of visits. Licensed practical nurses provide skilled nursing visits under the direction of the RN case manager. The Home Health Aide is assigned to help with personal care tasks as needed.
The professional social work staff at Hospice Chautauqua County is available to guide you and your family through the various stages of living with a life-limiting illness. The social workers can provide counseling, support, referrals and education to help you understand how to cope with illness, grief and loss.
