Heritage Park Rehab & Skilled Nursing, located on Prather Avenue in Jamestown, remains dedicated to providing the highest quality of short-term rehabilitative and long-term skilled nursing care to all residents, including the 149 residents who daily call Heritage Park, 'home.' However, it's not just physical needs that Heritage Park is meeting. Heritage Park strives to treat residents holistically - meeting the needs of the whole person. This includes those with memory impairments. A high percentage of Heritage Park's residents struggle with Alzheimer's and other forms of dementia. Through the assistance and training of Gillick Associates (Williamsville, N.Y.), Heritage Park is instituting 'Connections', a comprehensive memory care staff-training program, and creating a designated inpatient Alzheimer's/Dementia Day Care Center on the second floor of Heritage Park.
